- Be in good physical and mental health
- Have carried her own baby to term and have delivered at least one child naturally
- Have had pregnancies that were all free of complications and were full term

- Do not smoke or abuse alcohol
- Be committed to the entire process from beginning to end

Birth Records
Surrogates need to provide birth records from any previous pregnancies.
Medical Examination
Prospective surrogates are required to undergo a complete medical evaluation to check their blood type and hormone levels. This includes a pap smear, uterine lining test, STD (sexually transmitted disease) test, and more.
Criminal record
Prospective surrogates should have no prior convictions.
